At Hope Crossing it is our policy to provide essential services regardless of the client’s insurance coverage or ability to pay full fee. 

Our fees vary based on insurance benefits, counselor level, and income.

Several of our counselors on staff have contracts with a variety of insurance companies. 

We would be happy to help you navigate through your insurance benefits to determine what coverage and/or reimbursement you might have available. 

To help you determine your insurance benefits we would need some basic information:

  • Your legal, full name. 
  • Your birth date. 
  • Your relationship to the subscriber (insurance holder).
  • The name of your insurance company.
  • Your member ID from your card
  • Phone number(s) from the back of your card for either: "Mental or Behavioral Health", "Providers", or "Member Services"

If it is determined that your insurance will not cover our services there are still options we have for you:

For our clients whose insurance does not offer coverage our services or perhaps the deductible for out-of-network benefits is beyond your means available, we may be able offer a rate that is comparable your in-network benefits. 

We have several counselors who are working toward full licensure status who are able to offer this option. 

To determine the rate of your session we will request some basic information:

  • Your legal, full name.

  • Your relationship to the subscriber (insurance holder).
  • The name of your insurance company.
  • Your member ID from your card
  • Phone number(s) from the back of your card for either:  "Mental or Behavioral Health", "Providers", or "Member Services"

Once we gather your insurance information we will contact you (via email unless requested otherwise) to discuss your options.

Sliding scale fees are offered to clients who have no insurance coverage.

A completed application including required documentation of the home address, household income, and insurance coverage must be on file and approved by the business office before a discount will be granted.  Applications may be obtained at http://www.hopecrossingcc.com/storage/Financial%20Disclosure.pdf  or by contacting our business office.

Verification will typically include tax returns and current pay stubs. In addition to annualized income verification, eligibility may be based on current participation in certain federal/state public assistance programs, examples of which include the following:

  •  Social Security Income (Disability);
  •  Free or Reduced School Lunch Program;
  •  Other public assistance programs.

The mother, father, and dependent children under 18 are counted as the family. The income of all adults in the household is considered when evaluating the discount/sliding scale.  The discount status will be re-evaluated on an annual basis or when income/family status changes.  A "zero income narrative" is required if there is no income in the home.  (This consists of 1 paragraph explaining how you are meeting basic needs.)
